  • To try a new medication, we kindly ask that you book a check-up appointment to discuss with your provider. This ensures that you have the best options to address the reason(s) for making a switch. Please only use the form on this page for a refill of the same medication you currently have.

  • If it has been one year or longer since your last appointment, or if you are requesting another medication, you will need to schedule a check-up appointment. Additionally, certain scripts require more visits to monitor results and potential side effects.

  • With accurately provided information in the form above, and there are no follow up concerns from our providers, your script refill will be processed by our team within two business days. From there we encourage you to check with your designated pharmacy as to when it will be ready.
